Tile wear. Concrete tiles shed a fine sand as they weather and it settles into a dense layer that holds water against the base of the gutter and reduces the fall. Flushing does not shift it — it has to be cleared out.

Yes. We flush every downpipe and watch that it runs away rather than just clearing what we can see. On newer homes with large roof planes feeding few outlets, a partially restricted downpipe is a real risk and it will not be obvious until it rains hard.
